Health App Informed Consent

Purpose

Exodus Health lets you electronically record and review wellness, medication or peptide, lifestyle, appointment, and related health information and communicate with an authorized care team.

What you consent to

By checking the consent box during signup, you voluntarily consent to:

• creating and storing an electronic account and health-tracking record;
• entering health and wellness information and receiving app reminders or educational guidance;
• authorized Exodus Health providers and care-team personnel viewing information relevant to supporting your care;
• your identity, membership tier and status, contact details, and relevant member information being displayed to an authenticated provider when your member QR code is scanned; and
• electronic communications and records related to your use of the app.

Benefits, risks, and alternatives

Potential benefits include easier tracking and care coordination. Risks include inaccurate self-entered data, delayed or misunderstood electronic communications, technical outages, and the privacy risks inherent in electronic systems. You can instead track information offline and communicate with your provider through other available channels.

Important limitations

The app does not guarantee outcomes and is not a substitute for an in-person evaluation or your clinician’s instructions. Automated content is not a diagnosis or prescription. Do not use the app for urgent or emergency needs; call 911 or your local emergency number.

Voluntary consent and withdrawal

Your consent is voluntary. You may stop using the app or ask to withdraw consent and close your account. Withdrawal applies going forward and does not undo prior processing or records that must be retained by law or for legitimate clinical, safety, billing, or legal purposes. Ask your care team if stopping app use could affect services coordinated through it.
